- VA New Jersey Healthcare System (East Orange), New Jersey: 11 recommendations (report 25-00242-195)

Which VA hospitals are under investigation?
A healthcare facility inspection is not a criminal investigation. VA OIG inspects VA medical facilities on a routine cycle and publishes a report with recommendations for each one. The table above lists every 2026 Healthcare Facility Inspection report the OIG has published so far. Criminal cases the OIG investigates are listed separately on our VA fraud cases 2026 page.
Who is the VA Inspector General, and who does the VA OIG report to?
Cheryl L. Mason is the Inspector General of the Department of Veterans Affairs. VA OIG says she was confirmed by the Senate on July 31, 2025 and sworn in on August 4, 2025. The OIG describes its job as independent oversight of VA, and says it discloses risks it finds to veterans, the public, VA and Congress. It also sends Congress a Semiannual Report. Sources: Meet the Inspector General and Mission, Vision, and Values.
What does a VA OIG healthcare facility inspection look at?
2026 reports review areas such as culture, environment of care (cleanliness and safety), patient safety (including how test results reach patients), and access to care, including community care scheduling. Each report lists its recommendations. One recommendation can cover a single item, so a higher count is not a grade.
How do I file a VA OIG complaint?
Use the VA OIG Hotline. VA OIG says it accepts electronic complaints through its web forms. It also lists phone 1-800-488-8244 and mail: VA Inspector General Hotline (53H), 810 Vermont Avenue, NW, Washington, D.C. 20420. The hotline is not a crisis line. Veterans in crisis should contact the Veterans Crisis Line at veteranscrisisline.net.
